PROGRAM OVERVIEW
This program brings to life the underlying and interconnected pathophysiology that is responsible for cardiometabolic disease. Understanding how diabetes, heart disease, obesity, hypertension, and other metabolic disorders are interrelated provides important information when determining treatment pathways.
TARGET AUDIENCE
This educational activity is designed to meet the needs of healthcare providers who have a role in the diagnosis and treatment of patients with cardiometabolic diseases (CMDs), namely type 2 diabetes, chronic kidney disease, heart failure, and post-myocardial infarction complications. The multidisciplinary team members for which this activity is intended include primary care physicians, hospitalists, endocrinologists, associated advanced practice providers (including physician assistants and nurse practitioners), pharmacists, and certified diabetes educators.
LEARNING OBJECTIVES
Upon the completion of this program, attendees should be able to:
- Differentiate patients with high cardiometabolic risk factors or pre-existing CMDs through early screening of individual and concurrent CMDs based on clinical guidelines
- Craft evidence-based treatment approaches with CMD treatment, novel CMD drugs, and best practices for patients with CMDs
- Apply a multi-disciplinary approach, streamlined communication, and the patient’s perspective alongside shared decision making to optimize care for patients with CMDs
JOINT ACCREDITATION STATEMENT
