PROGRAM OVERVIEW
This clinical conversation symposium offers a dynamic forum for healthcare professionals to exchange insights and tackle real-world challenges in the management of chronic kidney disease (CKD) and heart failure (HF). Guided by expert faculty, participants will engage in case-based discussions that bring current guidelines and emerging evidence to life.
The program explores practical approaches to improving early identification, refining treatment strategies to achieve cardiorenal benefits, and integrating shared decision-making (SDM) into everyday practice. Collaborative care will be a central theme, with discussion focused on how multidisciplinary teams (MDTs) can work together to deliver coordinated, patient-centered treatment plans.
LEARNING OBJECTIVES:
After completing the CME activity, learners should be better able to:
- Apply guideline recommendations and CMD pathophysiology to aid in the screening, diagnosis, and specialty referral for patients with CKD and/or HF.
- Employ the latest clinical evidence and guideline recommendations to create comprehensive treatment plans that provide cardiorenal benefits for patients with CKD and/or HF.
- Practice SDM to incorporate the patient’s perspective in therapy decisions and collaborate with the patient’s MDT to create a comprehensive therapy plan when treating patients with CKD and/or HF.
TARGET AUDIENCE
This educational activity is designed to meet the needs of healthcare providers in primary care, including general/family physicians and/or internal medicine physicians (PCPs), primary care advanced practice providers (APPs; e.g., nurse practitioners and physician’s assistants), and endocrinologists who play a crucial role in diagnosing and treating patients with chronic kidney disease (CKD) and heart failure (HF) as a result of cardio-renal metabolic disease (CMD). This activity is also designed to provide education on the optimal care of CKD and HF through the multidisciplinary team (MDT), as well as evidence-based treatment approaches.
JOINT ACCREDITATION STATEMENT
